Efficacy of melatonin with behavioural sleep-wake scheduling for delayed sleep-wake phase disorder: A double-blind, randomised clinical trial
This trial investigated the efficacy of 0.5mg melatonin, combined with behavioral sleep-wake scheduling, in improving sleep initiation among individuals clinically diagnosed with Delayed Sleep-Wake Phase Disorder (DSWPD) characterized by delayed endogenous melatonin rhythm relative to desired bedtime (DBT) requirements.
The study involved 307 participants, with 116 receiving melatonin. Results indicated that individuals administered melatonin experienced a 34-minute earlier sleep onset compared to the placebo group. Melatonin treatment led to increased sleep efficiency at the first attempt to sleep (SE T1), along with reduced PROMIS sleep-related impairment, PROMIS sleep disturbance, insomnia severity, and functional disability. A lot of patients had more than minimal clinician-rated improvement following melatonin treatment (52.8%) compared to placebo (24.0%).
The findings demonstrate that administering melatonin 1 hour before DBT, in conjunction with behavioral sleep-wake scheduling, effectively improved both objective and subjective measures of sleep disturbances and related impairments in individuals with DSWPD exhibiting a delayed circadian phase relative to DBT.
