Etoricoxib Tablets are a non-steroidal anti-inflammatory drug (NSAID) used to reduce inflammation and relieve pain. It is indicated for the treatment of chronic and acute back pain and many types of arthritis, such as rheumatoid arthritis, osteoarthritis, gouty arthritis, and ankylosing spondylitis.

Etoricoxib Tablets are sometimes used to treat post-operative pain associated with dental surgeries. It is also occasionally prescribed to relieve cramp-like pain associated with menstrual periods.

Etoricoxib is a COX-2-selective inhibitor that is classified within the NSAID family. It works by blocking the action of the enzyme called cyclooxygenase-2 (COX-2). COX-2 is associated with the development of prostaglandin, which causes pain and inflammation in the body. However, unlike many other NSAIDs, etoricoxib does not block the action of COX-1, which is an enzyme that is helps protect the stomach from ulcers.

Etoricoxib Tablets are produced with a dosage of 60mg, 90mg, or 120mg. The tablets are packaged on blisters and available as a box of 14 or 100 tablets.

How should Etoricoxib Tablets be used?

This medication is intended to be taken orally. Etoricoxib Tablets should be taken with a glass of water and they may be taken with or without food.

Taking them without food may speed up the effect of the medication.

What is the difference between Etoricoxib and Naproxen?

Etoricoxib and naproxen are both classified as NSAIDs and are commonly used to relieve pain and inflammation associated with conditions like arthritis. These drugs are generally well tolerated by patients and have a similar level of efficacy. However, these two drugs may cause different side effects in patients. Etoricoxib is associated with a lower risk of upper gastrointestinal complications, but naproxen has less of an effect on blood pressure.

How should Etoricoxib Tablets be stored?

It is recommended to keep the medication in its original packaging until use. It can be kept in a location that is dry and out of direct sunlight. Factors such as excessive heat or dampness can compromise the integrity of this medication. Keep Etoricoxib Tablets out of the reach of children. Before use, check the expiry date and discard any outdated medications. Do not dispose of this medication in wastewater or household trash.

What dose should be taken?

Adult Dosing Dosage may vary based on different medical indications:

  • For osteoarthritis, the usual dose is 30mg taken once per day. If necessary, the dose can be increased to a maximum of 60mg per day.
  • For rheumatoid arthritis, the usual dose is 60mg once per day. If necessary, the dose can be increased to a maximum of 90mg per day.
  • For ankylosing spondylitis, the usual dose is 60mg once per day. If necessary, the dose can be increased to a maximum of 90mg per day.
  • For acute pain conditions, the usual dose is 30-60mg. Etoricoxib should be used only for acute pain conditions, then it is recommended to switch to another pain reliever if necessary.
  • For gout, the usual dose is 120mg taken once per day. The maximum duration of treatment is 8 days. Etoricoxib should only be used for the acute pain period of a gout flare-up.
  • For postoperative dental surgery pain, the usual dose is 90mg taken once per day. The maximum duration of treatment is 8 days.
  • For primary dysmenorrhoea, the usual dose is 120mg taken once per day. This drug should only be taken during the acute pain period. The maximum duration of treatment is 3-8 days.

Renal Dosing For patients with CrCl < 30ml/min, this medicine is not recommended.

Hepatic Dosing For patients with mild hepatic impairment, a maximum dosage of 60mg per day is recommended. For patients with moderate hepatic impairment, a maximum dosage of 30mg per day is recommended. For patients with severe impairment, this medicine is not recommended.

Pediatric Dosing This medication should only be taken by adolescents over the age of 16. See adult dosing for these patients.

Refer to a doctor or pharmacist for guidelines on dosage. Do not exceed what they advise. Dosage and duration guidelines may vary from region to region.

What happens if a dose of Etoricoxib is missed?

If a dose is forgotten, the patient should resume their normal schedule the following day. Never take 2 doses to make up for a missed dose.

Who can use Etoricoxib?

Etoricoxib Tablets can be given to adults and children over 16, but caution is advised for specific groups of patients.

Pregnant This drug should not be used in pregnancy. After 20 weeks of pregnancy, there is a potential for fetal harm. NSAID use during pregnancy is associated with an increased risk of fetal harm such as ductus arteriosus premature closure, renal impairment, and persistent pulmonary hypertension.

Breastfeeding It is not advised to give this medication to women who are breastfeeding. It is unknown whether this drug passes in milk.

Children This medication is not recommended for children under the age of 16.

Geriatric It is advised to express caution when prescribing this drug to elderly patients. Though this medicine is effective for patients over 65 years old, this population has an increased risk of adverse effects.

Other warnings

This medication can cause upper gastrointestinal complications and should be used with caution in patients who have a risk of GI complications due to NSAIDs, those who are currently taking other NSAIDs, those with a history of GI diseases, and the elderly.

Etoricoxib has been associated with an increased risk of thrombotic events, which may be dependent on the dose and duration of treatment. For this reason, it is advised to use the lowest dose and shortest duration of treatment.

For patients with cardiovascular risk factors, such as hypertension, smoking, diabetes, and hyperlipidemia, caution should be expressed when deciding to treat with this medication.

It is recommended to monitor renal function in patients with severe renal impairment, uncompensated heart failure, or cirrhosis.

This medication may cause more fluid retention and hypertension than other NSAIDs, so it is important to carefully address the risks and benefits for patients with pre-existing edema, a history of cardiac failure, left ventricular problems, or hypertension.

Side Effects

As with all pharmaceuticals, some unwanted effects can occur from the use of Etoricoxib Tablets.

Common side effects include, but may not be limited to:

  • stomach pain
  • fluid retention
  • feeling weak, tired, or dizzy
  • raised blood pressure
  • gut disturbances
  • palpitations or arrhythmia
  • wheezing
  • bruising
  • headache
  • constipation or diarrhea

  • difficulty with breathing

  • black or tarry stools, coughing up blood, or severe stomach pain

Precautions

Do NOT use Etoricoxib Tablets if:

  • You are allergic to etoricoxib or any of the ingredients.
  • You are allergic to NSAIDs, acetylsalicylic acid, or COX-2 inhibitors.
  • You have had a stroke or certain heart problems.
  • You have uncontrolled hypertension.
  • You are having surgery soon, particularly on the heart or arteries.
  • You are pregnant or breastfeeding.
  • You have severe liver or hepatic disease.
  • You have an ulcer or bleeding in your stomach or intestines.

Before treatment, consult your doctor regarding any medications you are taking to address potential drug interactions.Etoricoxib is known to interact with many drugs, including anticoagulants, antibiotics (rifampicin), methotrexate, ciclosporin, tacrolimus, diuretics, hypertension and heart medications, digoxin, minoxidil, salbutamol (oral), hormonal birth control, acetylsalicylic acid, and other NSAIDs.

This medication may not be suitable for people with certain conditions, so it is important to consult with a doctor if you have any health conditions, such as:

  • history of ulcers or stomach bleeding
  • dehydration
  • edema
  • heart problems
  • hypertension
  • liver or kidney disease
  • current infection
  • diabetes
  • high cholesterol
  • smoker
  • pregnant
  • elderly

Some patients have reported drowsiness and dizziness after taking this medication. It is recommended to avoid driving or operating machinery until it is clear how the drug affects you.

Etoricoxib is not suitable for children under 16 years of age.

Some formulations of Etoricoxib Tablets may include inactive ingredients such as lactose. It is advised to consult the packaging for a full list of inactive ingredients.

Evaluation of two doses of etoricoxib, a COX-2 selective non-steroidal anti-inflammatory drug (NSAID), in the treatment of Rheumatoid Arthritis in a double-blind, randomized controlled trial

Etoricoxib is a non-steroidal anti-inflammatory drug (NSAID) and it is used for the treatment of rheumatoid arthritis (RA). It is efficacious at a dose of 90mg.

This is a 2-part, double-blind, placebo-controlled study in RA patients. Part I of the study was a 6-week, placebo-controlled phase designed to evaluate the effectiveness of etoricoxib 90mg and etoricoxib 60mg, each compared to a placebo and to each other. Part II was another 6-week period aimed at assessing the potential benefits of increasing the dose from etoricoxib 60mg to 90mg after the initial 6-week exposure to 60mg in Part I, compared to continuing a steady dose of 60mg throughout both parts of the study. The primary endpoints were the Disease Activity Score for 28 joints and C-reactive protein level (DAS28-CRP) index, and the Patient Global Assessment of Pain (Pain) score on a 0–100 mm visual analog scale (VAS) after 6 weeks of treatment in Part I. Adverse events were monitored throughout the duration of the study.

The study included 1404 patients. It was revealed that both etoricoxib doses were superior to placebo on both primary efficacy endpoints (p = 0.004 for 60mg and p = 0.034 for 90mg for DAS28-CRP; p < 0.001 for both doses for PGAP) in Part I.

The conclusion of this study is that both etoricoxib 90mg and 60mg are superior to placebo in relieving the symptoms of RA.
